Family Medical Associates of Raleigh provides comprehensive family medicine to patients of all ages. We were established in 1987 by Dr. Stephen I. Moore, III, who was active in our practice until his retirement in November 2015.

Patient-Centered Medical Home (PCMH) is a team-based healthcare delivery model that provides comprehensive and continuous medical care with the goal of improving the health of all patients. Our practice has been designated a Patient-Centered Medical Home since 2010.
The National Committee for Quality Assurance defines Patient-Centered Medical Home as a healthcare model that facilitates partnerships between individual patients, and their personal physicians, nurse practitioner, or physician assistant and, when appropriate, the patient’s family. Partnering with patients and their families requires fully understanding and respecting each patient’s unique needs and values. We support our patients in learning to manage their own care at the level that they find comfortable.
We coordinate our patients’ care across the broader health care system including specialists, hospitals, home health care, and community services and supports. Care is facilitated by electronic exchange of information to assure that patients get care when and where they need and want it. Our goal is to make it easy for patients to navigate, understand, and use information and services to meet their health care needs.
As a medical home practice, we provide shorter waiting times for urgent needs, early morning and early evening
